Position Responsibilities :
- In collaboration with department leadership, reviews and revises department policies / procedures as necessary. Ensures the appropriate distribution and education of department policies / procedures.
- Ensures the management of data collection, aggregation and dissemination across the continuum, specific to clinical, financial and operational performance for designated area of responsibility.
This includes, but may not be limited to, concurrent and retrospective chart audits as necessary.
- Maintains and monitors daily reports to ensure appropriate staffing levels based on unit of service standards.
- Oversee the daily coordination and the implementation of the Eldercare PACE Model of Care.
- Direct, supervise, hire, fire and evaluate the performance of direct reports, including clinical oversight, education, training, reporting requirements and competency evaluations of the Center C.N.A.'s.
- In conjunction with the Director and the Quality Improvement Manager, maintains the stability and reputation of Eldercare by ensuring that all activities and operations are performed in compliance with local, state, and federal contracts, regulations, laws, accreditation, protocol, licensing and certification requirements governing operations.
- Assist in program development and implementation of Eldercare's strategic plan, including short and long-term goals and budgets.
- Participants or assigns a designee to all interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24 hour care delivery.
Serves as alternate facilitator.
- Oversee and direct the coordination of home health, rehabilitative, long-term care and medical / specialty services according to participant care plans.
- Manages flow of the clinic, oversees appointment bookings, triaging and gate keeping in conjunction with primary care and staff.
Ensures timely and appropriate care of participants.
- Assists Director with evaluation of Eldercare's performance against national results and established service standards, clinical outcomes, utilization and other financial criteria.
- Establish and maintain positive working relationships with all members of the Eldercare team, the hospital and all Network Providers.
- Oversees and ensures signing off orders, ordering medications from pharmacy and disseminating orders and instructions to appropriate personnel.
Ensures implementation of physician orders.
- Communicate with weekend and after hour staff, following up appropriately. Participates in the on-call rotation.
- Promotes effective communication and collaboration between the clinic and members of the enrollment, activity and business office.
- Participates in Leadership meeting to review risk scoring, pharmacy benefit managers Part D reviews, utilization, QI directives, Key Performance Indicator reviews and other key metrics of Eldercare at least monthly;
addresses concerns with the Director from staff. Assists in improving these reviews and updating this process as needed.
- Conducts an in person comprehensive initial, regularly scheduled assessments and other nursing assessments of Eldercare enrollees, according to program protocols either directly or via delegation and oversight
- Perform or supervise general skilled nursing care to participants. Perform or supervise the administration of prescribed medications and treatments in accordance with nursing standards.
Assist in treatment, examination and testing of participants. Assist with all aspects of hospital, SNF or ALF admissions.
- Insures coordination of participant care with outside contracted service providers nursing homes, assisted living, lab, oxygen, etc.
- Identifies and develops action plans to improve Eldercare operations as related to the financial operations of PACE in collaboration with the director, interdisciplinary team, enrollment and MSMC departments.
- Identifies opportunities and strategies to improve department processes to meet the evolving needs of the participants served, Eldercare and the community.
Monitors and reports department quality, service and cost outcomes.
Supports the director in implementing objectives and recommending improvement that support the vision and mission of the organization.
Demonstrates awareness and support of department objectives during interactions with staff and customers.
- In coordination with and in support of the director, plans and conducts meetings, encourages open communication and verbalization of opinions in an attempt to identify and resolve problems within the department.
- Participates in hiring, reviews, competencies, discipline, and grievance handling to ensure optimal individual and team performance.
- Participates in staff development activities, interacting as needed with the director, customers, and others to continuously improve competency and effectiveness of the department.
- Identifies and addresses skills or behaviors requiring improvement, both at an individual level and at a department level.
- Provides coaching as needed to optimize performance and ensures optimal customer service.
- Engages in self-development in professional, and managerial areas; successfully completes leadership development curriculum.
- Adheres to standards, policies and procedures at Eldercare, the hospital and Departmental level.
- Promotes a service orientation in the performance of position duties and responsibilities and in interactions with participants, staff and visitors.
Qualifications :
- Graduated from accredited school of registered nursing., BSN preferred
- Current RN Licensure in State of Florida. BLS and CPR required.
- Minimum of three (3) years of health care experience preferable in a geriatric physician office required. Experience in nurse management in managed care, hospital, nursing home or urgent care is acceptable for this position.
A minimum of one year's experience working with the frail or elderly population is required. Either training or related experience in direction of a health care program.
