Registered Nurse Home Health - Travel - (HH RN)

StartDate : 11 / 18 / 2024 Available Shifts : 8 D Pay Rate : $2470.44 - $2722.53

Title : RN Home Health - Beantown / Chelsea Float. Unit level template : Bill Rate $100 / hr-Start ASAP for 13 weeks - biweekly orientation-Weekend rotation every 6 weeks -Travelers will be expected to float between the following PHH Teams : Marathon, Minuteman, Revolution, Paul Revere - (request breakdown of towns covered for each team by email) Home Health and Oasis experience requiredEPIC preferredEinstein Req# Send files to Christine and submit in Hallmark / Einstein This is a premium pay / 2.

0 OT facility. Skills : RN Home Health. Description : 8am-4 : 30 pm ALL APPLICANTS MUST HAVE 1 year Home Care experience.

1 year Case Management Experience. Ability to travel to cases as assigned. Job Summary : As a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.

Patient care is delivered within organizational philosophy, policy and standards of community health practice. Also required to perform admission processes and case management responsibilities in accordance with MGB Home Care’s policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
