Full Time Licensed Physical Therapist -Nursing Home
Located near Hempstead, NY
A skilled nursing facility located near Hempstead, NY is looking for an experienced and knowledgeable Full Time Licensed Physical Therapist.
Responsibilities :
- Provide patient care and treatment using various physical therapy techniques
- Assess and evaluate patients' conditions and develop individualized treatment plans.
- Collaborate with other healthcare professionals to coordinate patient care.
- Monitor and document patients' progress and adjust treatment plans, as necessary.
- Educate instruct family members and caregivers of the patient on their treatment plan, and discharge planning
- Maintain accurate and up-to-date patient records
Qualifications :
- Bachelor's or Master's degree in Physical Therapy from an accredited program.
- Valid state licensure as a Physical Therapist.
- Experience in acute and long-term care is preferred.
- Strong knowledge of medical terminology, anatomy, and physiology.
- Excellent communication and interpersonal skills.
- Ability to work independently as well as part of a team.
- Knowledge of NetHealth and Sigma care a plus
