Overview
Senior Outreach Therapists provide in-home therapy to adults, 60+ years. Therapy services include a psychosocial assessment, ongoing client therapy, community referrals, and client education regarding community resources and mental health issues.
Maintain reciprocal communications with not only referring workers, but possible referral sources and other collaborating resources in the service area.

Responsibilities
Client Services :
Provides in-home therapy to individuals aged 60+. Therapy services include developing a therapeutic relationship, provide support and encouragement with the individual, providing a psychosocial assessment, mental health diagnosis, providing ongoing therapy, and community referrals.
Capable of providing all services independently with only minimal supervision and consultation. Involved with managing Aging Resource Grant for funding.
Casework Documentation / Collateral Tasks :
- Insures timely progress notes, assessments, a client centered treatment plan with the client, discharges that adequately portray services delivered and client progress regarding those services.
- Documentation of scheduling, to include time worked, requested time off, and time off taken.
- Attend all required meetings and trainings.
Transportation to Job Sites :
- Accomplishes necessary driving tasks independently to client homes, temporary placements, or when providing mental health outreach to community resources and facilities.
- Demonstrates ability to build this into schedule to allow for production expectations and adequate time for client services.
Qualifications
Education : A Master's degree in Social Work or related field Required. Advanced training in providing therapy, treatment planning and diagnosis for mental health related services prefrerred.
Experience : Work experience in providing services to the mental health population required. Experience in gerontology, Experience with In-home therapy Experience in working in mental health setting preferred.
License(s) / Certification(s) : Licensed as a mental health provider in the State of Iowa : LMSW, LISW, LMHC, LMFT, tLMFT, tLMHC.
Knowledge / Skills / Abilities : Planning and organizing skills, Communication-written and verbal, Independent decision making, Customer focused, Basic computer skills, Proficient with Cerner and Excel, Knowledge of geriatric issues, Advanced knowledge in providing therapy to individuals.
Other : Valid driver's license when driving any vehicle for work-related reasons.
