Primary Function / General Purpose of Position
The RSFH Hospital at Home program delivers acute hospital-level care to patients in the comfort of their own homes. Patients meeting inpatient admit criteria for qualifying medical conditions, such as cellulitis or pneumonia, will be admitted into the Hospital at Home program upon successful completion of a program eligibility assessment.
The Hospital at Home Care Consultant is responsible for patient care including but not limited to pre-admission, social services, and post-acute service.
The Hospital at Home Care Consultant collaborates with the interdisciplinary healthcare team and patients / families / caregivers to expedite appropriate, cost effective, and safe quality care that optimizes patient flow and promotes an integrated and seamless patient-centered experience.
This position ensures holistic, evidence-based care plans are consistent with patient’s clinical course, continuing care needs, and covered services.
The Hospital at Home Care Consultant is a resource to both internal and external stakeholders and assists in the education and guidance of the care team and patients / families / caregivers in shared decision making.
The Hospital at Home Care Consultant is responsible for business development and promotion of RSFH Hospital at Home Program and post-acute services.
The Hospital at Home Care Consultant interfaces with the community as well as referred patients by coordinating the RSFH post-acute continuum of care.
The Consultant is responsible for gathering patient demographic data, medical history and physician documentation and appropriately processing the referrals for admission to the Hospital at Home program and post-acute setting in an expedited fashion.
Professionally represents RSFH at conferences, community activities and through daily interactions. Performs business development calls with referral sources, tours clients and interested parties through the facilities as needed.
The Consultant seeks opportunities to further develop the referral base and referral volume to RSFH Hospital at home program.
Minimum Qualifications
Education : Associates or baccalaureate degree required. Graduate from an accredited institution for either RN, LPN, PT, PTA, OT, OTA.
SLP, Recreational Therapy or SW preferred.
Experience : Minimum of two (2) years’ experience in a clinical practice setting with experience in discharge planning, case management, and / or intake / referral coordination.
Previous sales / marketing or business development experience is preferred.
Licensure / Certification : Must be licensed in the state of South Carolina for respective specialty. Must have current driver’s license.
Primary Source Verification (if applicable) : SC Licensure in either RN, LPN, PT, PTA, OT, OTA, SLP or SW that can be primary source verified by SCLLR or Nursing- compact RN state licensing board or certification in recreational therapy.
Knowledge / Skills : Must have exceptional customer service, communication and interpersonal skills. Demonstrates a thorough working knowledge of medical issues, post-acute federal regulations, and CMS / commercial insurance requirements.
Able to establish effective working relationships with staff, third party payers, referral sources, discharge planners, insurance case managers and physicians.
Must have moderate to advanced computer skills. Ability to analyze data generated by all referral activities. Must function independently and have strong organizational, negotiation, and critical thinking skills.
Contacts : All teammates of the RSFH post-acute continuum including physicians, discharge planners, administration, nursing, therapy, and medical records.
Additional contacts include insurance companies, patient accounting, families / caregivers, other hospital departments, community facilities and the general public.
