Description

The Physical Therapist is directly responsible for planning and administering medically prescribed physical therapy treatment programs in the home settings for patients to restore function, relieve pain and prevent disability following disease, injury or loss of body parts.

The physical therapist consults with the physician in developing a plan of treatment including specific procedures and equipment to be used and frequency of visits.

Required Qualifications :

  • Degree in Physical Therapy
  • Washington Physical Therapist License upon hire
  • National Provider BLS - American Heart Association within 30 days of hire
  • National Provider Identifier (NPI) number within 30 days of hire
  • 1 year Home Health
  • This position requires that you obtain an NPI number within 30 days of starting in the position. The NPI process is free, and the application may take several weeks to process.

Preferred Qualifications :

  • 2 years of Physical Therapy with adults with physical disabilities
  • 1 year of experience with the geriatric population

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
