JOB SUMMARY

The Mental Health Specialist / Case Manager IV (MHS) reports and works collaboratively with the Supervising MHS. The MHS is responsible for providing services to adult offenders living with severe mental illness in collaboration with the Fresno County Probation Department.

The MHS is expected to possess strong communication, rapport building, boundary setting and problem-solving skills. The MHS will provide rehabilitation, case management, placement, and crisis intervention services.

The MHS will focus on supporting clients with their individualized wellness goals. Applying a collaborative person center approach is essential to the MHS position.

The MHS provides support through rapport, compassionate care, education, role modeling, advocacy, skills building, and linkage to Medi-Cal, social, financial, medical, housing, and other services.

ESSENTIAL JOB RESPONSIBILITIES

  • Assist in the accomplishment of the client Plan of Care (POC) under the clinical direction of the Mental Health Professional and supervision of the Supervising MHS.
  • Collaborate with clients to develop an individualized Service Plan (ISP). The plan will include written details of the supports, activities, and resources required for the individual to achieve their personal wellness goals.

Special care will be taken to document the client's personal choices / preferences, criminogenic, safety, financial, physical, mental, and behavioral health care needs.

  • The Mental Health Specialist / Case Manager will be assigned a caseload of criminal justice involved adult clients living with Severe Mental Illness (SMI) and co-occurring disorders.
  • Participates in various aspects of the Mental Health Diversion Court (MHDC) and / or any additional Behavioral Health Court including appearing in court as an agency representative and coordinate services
  • Maintain and submit timely documentation of client services / progress per Medi-Cal documentation guidelines.
  • Advocate and communicate closely with agencies, vendors, management, clients and significant others involved in the client care.
  • Consistently meet program performance standards including a service impact of 65%, sign and submit documentation goals, and attainment of 40 hours of job related in-service training per year.
  • Attend daily team check-in meetings, weekly one-on-one meetings with the Supervising MHS, and critical bi-monthly meetings including staff meetings, probation case staffing, and high risk resource team (HRRT) meetings.

Provide effective and thorough feedback regarding client engagement, progress, barriers, and needs.

  • Provide direct individualized services as outlined in the POC and individual service plan. May facilitate group services as needed under the direction of the Supervising MHS.
  • Provide the Supervising MHS with timely reports and updates including any key communications between community partners, vendors, clients, or unusual occurrences that might be relevant to program operation or the health and safety of clients or employees.
  • Will drive on Agency business.
  • Physical presence in the office is required.
  • Exercise sound judgment when providing client services, prioritize safety, manage time efficiently to meet deadlines, and respond quickly / effectively in emergencies or pressured situations to support de-escalation.

ESSENTIAL JOB REQUIREMENTS

  • Bachelors' degree in social work or counseling in a related field and one (1) year experience working in a similar or related field.
  • Communicate effectively in written and spoken English.
  • Work effectively with diverse ethnic, religious, gender, sexual orientation, age, socioeconomic and professional groups of people.
  • Ability to operate within Electronic Health Records (EHR) system.
  • Current certified training in emergency first aid procedures and CPR, preferred.
  • Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication / meeting platforms.
  • Possess a valid driver's license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws.

Agency insurer authorization is required.

  • Ability to pass a criminal background check.
  • Ability to pass a pre-employment physical, drug screen, general physical, and TB test.

ESSENTIAL JOB REQUIREMENTS FOR BILINGUAL SKILL

  • Communicate effectively in written and spoken Spanish.
  • Required ability to communicate effectively with monolingual Spanish speaking clients and families for all clinical purposes.
  • Assist with phone calls for Spanish speaking clients.
  • Required to provide language translations for case managers and Physicians on a periodic basis.
  • Minimal adequate competency in the Spanish language as evidenced by successful passage of the TPOCC Spanish Competency Test.
  • Other translation tasks as required.

  • Monolingual : Class 130 ($21.13 - $27.81 / hour)
  • Bilingual : Class 135 ($22.21 - $29.23 / hour)
