About The Position
Job Details
Schedule :
Monday to Friday, 8 : 30 AM - 5 : 00 PM
Responsibilities :
- Providing hospice social work services
- Evaluating patient care needs and developing plans to improve quality of life
- Communicating with the home health and hospice care team
- Maintaining accurate assessment and progress documentation
Additional Information :
- 13-week assignment starting immediately
- Minimum of 2 years of recent experience in home health, hospice, or palliative care preferred
- Required certifications : MSW (Master's in Social Work), CPR, BLS; LCSW considered if the candidate has a California license
- Patient population : Adults, primarily ages 50 and older, with a caseload of 4-5 patients per day
