• Lead with Excellence : Oversee the Hospice program in Middlesex County, ensuring seamless coordination and delivery of patient care to uphold the highest standards of operational excellence.
• Drive Patient Care : Manage a caseload of hospice patients, offering direct care and support, and making a tangible difference in their lives.
• Expand Community Reach : Conduct outreach activities to improve community access to hospice care and enhance awareness of our services.
• Ensure Quality : Monitor and evaluate the quality and appropriateness of services, striving for exceptional patient satisfaction.
• Foster Collaboration : Build positive relationships with co-workers and foster a collaborative work environment, all while striving for excellence in every aspect of your role.
