Position Summary : The Clinical Specialist, Prevention is the primary point of contact for prevention families in need of care coordination.

This position provides clinical support and expertise for children and families served through diversion and prevention programming and is responsible for coordination of behavioral health services for children enrolled in the Child Welfare Specialty Health Plan through Sunshine Healthcare.

This position coordinates community mental health, clinical and substance abuse treatment services and discharge planning for any child hospitalized or admitted to an inpatient facility.

ESSENTIAL FUNCTIONS :

  • Care Coordination Essential Function : Primarily responsible for supporting children and youth dually involved in APD, DJJ, substance abuse, mental health, post-adoption and diversion families with care coordination.
  • Coordinating Supports Essential Function : Assist with monitoring and coordinating supports and services for families involved with diversion and prevention (Family Support Services cases).
  • Clinical Consultation Essential Function : Provide clinical consultation in cases involved in diversion and prevention services.

This includes completing assessments and screenings on children to address level of care and needs, including those related to suspected involvement in human trafficking.

  • Multi-Disciplinary Team Staffing Essential Function : Coordinate and facilitate Multi-Disciplinary Team Staffing’s (MDT’s) on high-risk and human trafficking cases, as required.
  • Conduct Local Review Team Meetings Essential Function : Actively participate in, and facilitate, as needed, Local Review Teams and other applicable staffing / meetings.
  • FSFN Documentation Essential Function : Document activities and actions in Florida Safe Families Network (FSFN).
  • Maintain Data Essential Function : Maintain diversion, adoption, and clinical data for reporting purposes, to identify gaps in services, for resource allocation, and for forecasting needs.
  • Annual Reviews Essential Function : Participate in creation and annual reviews of the diversion and post-adoption policies and procedures to reflect effective practices that adhere to current state, federal, and system of care requirements.
  • Quality Improvement Essential Function : Participate in all diversion and prevention quality improvement processes.
  • Other duties as required : Job performance requires fulfilling other incidental or related duties as assigned, assisting, and training others, and performing duties of higher rated positions from time to time for developmental purposes.

QUALIFICATIONS : REQUIRED :

REQUIRED :

  • Master’s degree in social work or mental health counseling
  • Minimum of 2 years of experience working with children with behavioral, mental health, or developmental disorders
  • Minimum of 2 years of experience in child welfare

PREFERRED :

  • Licensure preferred as LMHC, LCSW, or LMFT
  • Knowledge of funding sources and provider resources available in central Florida.
  • Familiar with services and community resources available to serve individuals with substance abuse, mental health, developmental disabilities, and delinquency issues.
  • Completed the Adoption Competency and Human Trafficking training. This must be completed within the first 90 days of employment.
