Job Summary : The Certified Home Health Aide (C.H.H.A.) Provides direct resident care to ensure residents plan of care is properly followed.
The C.H.H.A. completes all required documentation as per state and federal guidelines. The C.H.H.A. also abides by all facility policies and procedures.
Essential Job Functions (List in order of importance) : Demonstrates ability to provide appropriate resident care.Provides resident care as per Resident Service Plan;
including the following tasks : ROM exercisesAmbulationTransferADLsAssisting with meals and feedingBed makingPersonal HygieneSkin ChecksAM and HS careVital signsAnswer pendants promptlyPostmortem careAssist with room changesParticipate in reportsLaundry ServicesAssist with braces and special stockings Ensures that all assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shift hours.
Reports any unusual changes in resident condition to Charge Nurse. Communicates pertinent information and actual or potential problems to charge nurse throughout the shift.
Completes all required documentation accurately and timely as per facility / state / federal policies. Monitor meal and room service.
Vitals signs recorded as assigned. Participates in resident conferences and contributes to Service Care Plan of a resident as requested.
Encourages residents to socialize and participate in activities. Provides reality orientation and motivation when necessary.
Shares information, ideas and personal skills with other team members in the department and facility. Asks if other team members need help.
Delivers the highest level of service to all residents, families and visitors. Follows the Meridian Way and Rules of Service to ensure satisfaction.
Handles special projects and rush assignments as needed, in a timely manner.Participates on committees as requested by supervisor.
Monitors patients condition by observing physical and mental condition. Helps patients care for themselves by teaching use of a cane or walker, special utensils to eat, special techniques and equipment for personal hygiene.
Maintains a safe, secure and healthy community member environment by following asepsis standards and procedures; maintaining security precautions.
Updates job knowledge by participating in educational opportunities. Knowledge, Education and Skills Required : High School diploma or equivalent.
Nursing Aide Certification (C.N.A.) OR Certified Home Health Aide (C.H.H.A.). Knowledge, Education and Skills Preferred : Previous nursing assistant experience preferred.
