As a Clinical Dispensing Home Infusion Pharmacist, you will review charts and lab results for an assigned caseload of patients receiving specialized, complex infusion therapy and TPN care.

You will communicate routinely with the patient and caregivers; answering questions, gathering information and discussing updates and course recommendations with the physician.

You will utilize your excellent written and verbal customer service skills as well as advanced computer skills in order to interact with key caregivers and patients.

Our Coram branch in Henderson, NV is searching to add a part time home infusion pharmacist to our team. The part time pharmacist will work 3 days (up to 24 hours) per week Monday to Saturday and participate in our on-call rotation.

On-call usually consists of taking evening calls1-3 days per week and 1 weekend per month

Key responsibilities :

• Building strong one-on-one relationships with your patients while they are recovering or receiving routine treatments, and supporting remote patients on a routine schedule and as needed.
• Participating in patient evaluation and chart review for acceptance, product selection, device selection and supply management.
• Collect, interpret and evaluate all physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness.
• Monitor the patient’s progress and assessment results to communicate clinical and technical information with the physician and other members of healthcare team.

Most patient contact will be on the phone and, on occasion, you will meet them in our infusion suites.

• Directly supervise pharmacy technicians and support personnel in the appropriate compounding, packaging and labeling of compounded sterile products.
• Assist in training and orientation of new employees, performance improvement activities and educational programs
• Communicate clinical and technical information and monitoring to other members of healthcare team and patient when appropriate
• Initiates, reviews and updates all required documentation
• Performs accurate computer order-entry, labeling and dispensing in accordance with policies and procedures and state board of pharmacy regulations
• Accurately compounds and checks compounded sterile products
• Maintains procedures for appropriate security of all legend items, including controlled substances
• Participate in on call rotation as indicated by staffing and business needs
• Experience in pricing / underwriting for Pharmacy and or home infusion, enteral nutrition, TPN
• Aligns with the Company’s policy on the allowable clean room garb and dress code requirements

Required Qualifications

• Active Registered Nevada Pharmacist License
• Communication and organizational skills
• Ability to work with a multidisciplinary team - doctors, nurses, pharmacists, technicians, and sales team
• Ability to multi-task

Preferred Qualifications

• 1 year of home infusion or hospital therapy experience
• Leadership experience
• experience with pharmacy software (EPIC)

Education

Graduated with a Doctor of Pharmacy (PharmD) or Bachelor's of Pharmacy Degree

Pay Range

The typical pay range for this role is :

$38.63 - $83.17

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.

The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.

This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
