Job Description :
- Provide comprehensive social work services to patients and their families, including psychosocial assessments, counseling, and discharge planning.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Advocate for patients’ rights and coordinate community resources to ensure continuity of care.
- Maintain accurate documentation in compliance with regulatory standards and hospital policies.
- Participate in staff meetings, case conferences, and ongoing professional development activities.
Education and Certification Requirements :
- Master’s degree in Social Work from an accredited institution.
- Current licensure as a Social Worker in the state of Arizona.
- Minimum of 2 years of clinical social work experience preferred.
- BLS certification required.
