DUTIES :

• The Renal Social Worker uses clinical skills inrendering treatment and referral services to all End Stage RenalDisease patients and their families directed towards the objectivesof improved patient care and the meeting of therapeutic goals.
• Conducts psychosocial assessment.
• Document on patient status on a quarterlybasis and PRN.
• Actively participate in thepatient care plan meetings.
• Interpretsfacility policies and procedures to patients and families whenappropriate.
• Participates in inservicetraining in the facility.
• Compiles anddevelops educational material and uses them as aids in psychosocialeducation.
• Initiates referral of specificpatients to community agencies for followup psychosocial care.
• Approaches the patients families and allvisitors in a courteous friendly manner with respect to theirrights and concerns.
• Reduce patientsfamilies and visitors anxieties by introducing self explaining allprocedures and possible delays in schedule orienting them to thefacility.
• Maintain confidentiality andprivacy of all patients.

REQUIREMENTS :

• This positionrequires a Licensed Master Social Worker (LMSW) or LicensedClinical Social Worker (LCSW) who is licensed by the State of NewYork.
• Holds a masters degree in social workfrom a school of social work accredited by the Council on SocialWork Education.
• One year of dialysisexperience preferred but willing to train the right candidate.
