Missouri Licensed Nurse Case Manager

A company is looking for a Nurse Case Manager II.

Key Responsibilities

Utilizes a collaborative process for assessment, planning, facilitation, and advocacy to meet individual health needs

Conducts comprehensive assessments of referred members' needs and eligibility for case resolution

Applies case management processes in compliance with regulatory and accreditation guidelines

Required Qualifications

RN with current unrestricted state licensure

3 years of clinical practice experience, preferably in home health or ambulatory care settings

Case Management Certification (CCM) preferred

Proficiency in computer skills

Ability to work in a fast-paced environment
