The Yukon-Kuskokwim Health Corporation (YKHC) is located in the Southwestern Alaska town of Bethel, with a total of approximately 6,000 people.
YKHC supports 1500 employees, providing the health services to a primarily Yup'ik Eskimo Native population for an area the size of Oregon.
Services are provided through a Bethel located hospital and 50 clinics throughout the YK Delta.
Position Summary :
The goal of case management is to ensure that women receive timely and appropriate re-screening, diagnostic, and treatment services.
Therefore, the position is accountable for assessment, planning, coordination, monitoring, evaluation, and resource development for patients with breast and cervical health needs.
These elements represent a cooperative process between the client and provider intended to assure timely re-screening, diagnostic, and treatment services.
The Case Manager assists the Program Director in planning, coordinating, and providing culturally appropriate women’s health promotion activities within the YKHC service area, coordinating activities with Health Education, and other educational organizations as appropriate.
The Case Manager also provides assistance in professional outreach activities for providers to keep informed on breast and cervical health screening updates, technologies, and management of abnormal findings.
Position Qualifications :
- Minimum Education : Must have completed a program of education in nursing from an NLN institution. Associates of Science in Nursing required, BSN preferred.
- Minimum Experience : 3-5 years prior experience in nursing.
- Licensing / Certification : In addition, must be licensed in good standing in the State of Alaska. Current BLS. The successful candidate will be required to pass and maintain competencies for this position.
- Equipment / Tools : Office machines (i.e., multi-line telephones, computers, copy machines, etc)
- Specialized Knowledge and Skills :
- Knowledge of medical terminology.
- Ability to work in patient care setting
