A company is looking for a Clinical Pharmacist to provide patient care and support clinical teams in medication management and chronic disease support.
Key Responsibilities
Provide one-on-one counseling to patients and caregivers on medication use and adherence
Collaborate with healthcare teams to develop patient-centered care plans and educate on safe prescribing practices
Utilize clinical data to identify high-risk patients and support quality improvement initiatives related to medication management
Required Qualifications
Doctor of Pharmacy (PharmD) degree required
Active pharmacist license in the State of OK and IL preferred, or ability to obtain a license in these states
Minimum 3 years of clinical pharmacy experience, preferably in ambulatory care or population health
Direct patient care experience required; background in FQHCs or working with underserved populations strongly preferred
Familiarity with value-based care and chronic disease management
