Primary Responsibilities
- Provide compassionate and personalised clinical social work services to patients in a post-acute setting
- Collaborate effectively with an interdisciplinary healthcare team to enhance patient outcomes
- Utilise Electronic Medical Records (EMR) accurately and efficiently
- Maintain empathetic and professional communication with patients and their families
Required Qualifications and Skills
- Master of Social Work (MSW) degree from an accredited institution
- Valid LCSW license in good standing in Virginia
- Proficient in the use of EMR systems
- Exemplify compassion, intelligence, motivation, and teamwork skills
