Salary : $88,000 - $101,000 + mileage reimbursement

Schedule : Monday through Friday

Hours : 8 : 00am 5 : 00pm

We're in search of a Hospice RN Case Manager to join our close-knit team at a mid-sized company that prioritizes a welcoming and familial atmosphere.

Qualifications :

• Licensed to practice as an RN in the state of MN. License is active and in good standing.
• Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate.
• 1 year of hospice care experience preferred.
• Able to travel within geographic area serviced by office. Valid driver’s license, and proof of auto insurance is required.
• CPR Certification

• NE North of Highway 94 and East of Highway 65
• SE South of Highway 94 and East of Highway 35W
• SW South of Highway 394 and West of Highway 35W

• South : Scott, Dakota
• East : Washington
• Mid : Anoka, Hennepin, Ramsey

Schedule :

• Full-time position with shifts during the day, from Monday to Friday. Occasional holiday work (two holidays annually) and the possibility of weekend shifts based on staffing requirements.
• Approximately 5 patients a day, average caseload is about 15.

Responsibilities :

• Will provide in-home Hospice care to our patients.
• Conducts an initial and thorough evaluation of the patient and their family to ascertain hospice requirements. This assessment encompasses an exploration of their present and prior medical conditions, alongside a physical examination of the patient with a particular focus on pain and symptom management.
• Performs continuous evaluations of the patient's physical condition and the psychosocial well-being of both the patient and their family.
• Commences and formulates a care plan in cooperation with the Interdisciplinary Group (IDG), the patient and their family, as well as the attending physician.
• Makes essential adjustments to the IDG care plan as the patient's or their family's condition and needs evolve.
