Education and Qualifications
- EDUCATION AND EXPERIENCE : Bachelors or Masters Degree in occupational therapy
- EDUCATION AND EXPERIENCE : One to two years related experience and / or training, or equivalent combination of education and experience
- LICENSES OR CERTIFICATIONS : Current California Occupational Therapy License
- LICENSES OR CERTIFICATIONS : Current CPR course completion
- QUALIFICATIONS : A valid California drivers license when travel is required
- QUALIFICATIONS : Able to organize and monitor the work of others
- QUALIFICATIONS : Able to work independently with minimal supervision
- QUALIFICATIONS : Analytical and diagnostic ability capable of interpreting patient physicians orders for occupational therapy patients and developing an effective treatment plan as needed

- PHYSICAL REQUIREMENTS (b) : This job requires frequent bending, squatting, kneeling, climbing, reaching above shoulders, sitting, walking inclines and declines, standing, talking, hearing, and performing repetitive hand motions.
Vision requirements include close vision and the ability to adjust focus. The employee will occasionally pull, lift, transfer, or push a patient side to side, to a sitting position, to a standing position, or laterally.
The employee must demonstrate a pull lift of 50 pounds from the floor.
POTENTIAL EXPOSURE TO BLOOD & BODY FLUIDS : Category 1 for potential exposure to blood body fluids. (Requires the performance of procedures or other tasks in the work routine that involve an inherent potential for mucous membranes or skin contact with blood, body fluids or tissues, or a potential for spills or splashes of them.
Protective measures should be used while engaged in Category 1 tasks with exposure
Job Roles
Occupational Therapist (AHCL)
- Accepts start of care assignments and schedules visits within agency established timeframes. Takes an active part in the scheduling of patients so as to be as efficient as possible.
- Applies diagnostic and prognostic procedures to evaluate the patients level of function in activities of daily living including eating, dressing, food preparation and other household activities.
- Assists in orientation of new personnel.
- Assists with ongoing collaborative interdisciplinary planning to ensure high quality home health care services.
- Assumes case management responsibilities when designated.
- Directs and assists patients in prevocational evaluation and follow-up.
- Documents all education, intervention actions and coordination of care clearly and concisely in the medical record according to policy and format in a timely manner, according to Medicare standards and departmental policies.
- Ensures reimbursement for care provided by submitting proper documentation and obtaining authorization.
- Exchanges information with other professional team members as appropriate. Guides the patient in the use of therapeutic, creative and self-help activities for the purpose of improving function.
- Implements the plan of care with the participation of the patient / family / caregiver to facilitate desired outcomes through therapeutic, preventive and rehabilitative interventions.
- Instructs and supervises other health care personnel, patients and family / caregivers when appropriate.
- Is familiar with Medicare / Medi-CalHMO insurance guidelines for provision of patient care.
- Maintains and meets expectations on time for all competencies, license, certifications and education requirements as outlined by local administration, Adventist Health (AH), The Joint Commission (TJC), Centers for Medicare and Medicaid Services (CMS), and all other regulatory agencies.
- Observes, records and reports to the physician the patients reaction to treatment and any changes in the patients condition.
- Participates in required staff meetings, case conferences, Inservice programs, utilization review and team conferences.
Participates in professional development activities to enhance professional growth and facilitate patient care.
Participates in the development, reassessment and revision of the medical plan of treatment on an ongoing basis as medical / environmental changes indicate and with participation of patient, physician, family, caregivers and significant others.
Performs additional duties as assigned.
- Performs ongoing evaluation of patient / family / caregiver responses to interventions, thereby determining progress toward goal attainment and / or revision in the plan of care.
- Plans for patient discharge and discharges patient appropriately.
- Provides for home health aide supervision.
- Reports pertinent patient assessment and evaluation data to physician / case manager / clinical supervisor, in a timely manner.
- Screens appropriately for referrals to other disciplines.
- Willingly performs other duties and innovations as assigned.
