Colorado Licensed Practical Nurse

A company is looking for a Remote Licensed Practical Nurse - LPN - LVN - Intake Coordinator.

Key Responsibilities

Conduct preliminary health assessments for newly enrolled patients

Resolve patient inquiries and facilitate open communication to understand their needs

Identify and coordinate community resources to benefit patient care

Required Qualifications, Training, and Education

Active Compact / Multi-State license as an LPN or LVN

At least 3 years of direct patient-facing work experience

Technical aptitude with Microsoft Office Suite

Ability to work in a fast-paced environment

Experience in chronic care management is preferred
