Position Overview : We are seeking a compassionate and experienced Social Worker to join our hospice and palliative care team.
In this role, you will provide emotional support, counseling, and resource coordination to patients and families facing serious illness.
As a vital member of the interdisciplinary care team, you will advocate for the psychosocial needs of patients, ensuring they receive comprehensive and holistic care that honors their dignity and wishes.
Responsibilities :
Provide psychosocial assessments for hospice and palliative care patients, identifying emotional, social, and financial needs.
Offer counseling and emotional support to patients and their families, helping them navigate the challenges of end-of-life care.
Collaborate with the interdisciplinary care team, including physicians, nurses, chaplains, and other professionals, to develop and implement individualized care plans.
Assist patients and families with advance care planning, including living wills, DNR orders, and other end-of-life decision-making processes.
Connect patients and families with community resources, such as grief support groups, financial assistance programs, and other services to support their care journey.
Educate patients and families about hospice and palliative care services, ensuring they understand their options and the support available to them.
Advocate for patients’ rights and help address any concerns related to care, treatment, or living arrangements
Document patient interactions, progress, and care plan updates in accordance with regulatory and organizational guidelines.
Participate in team meetings, case conferences, and care reviews to ensure comprehensive care delivery.
Qualifications :
Master’s degree in Social Work (MSW) from an accredited institution.
Licensure as a Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW), depending on state requirements.
Minimum of 1-2 years of experience in hospice, palliative care, or a related healthcare setting
Strong knowledge of end-of-life issues, grief counseling, and advance care planning.
Excellent communication and interpersonal skills, with a compassionate and empathetic approach to care.
Ability to work collaboratively within an interdisciplinary team.
Strong organizational skills and the ability to manage multiple cases effectively.
Valid driver’s license and reliable transportation.
