JOB DUTIES & RESPONSIBILITIES :
Locations : Albany / Camilla / Cordele / Jesup / Pelham / Unadilla / Vienna / Tifton / Valdosta
- Insure client supervision, health, safety, development and quality of care according to client's Individualized Service Plan (ISP) or other care plan.
- Provide constant care and aid to clients including but not limited to bathing, grooming, dressing, toileting, assisting with eating and hydration, skin care, positioning and turning.
- Assist clients when moving in and out of beds, baths, wheelchairs and automobiles.
- Assist clients with medication administration to ensure compliance with written physician orders.
- Document client activities, care, services, medication, vital signs, and other occurrences throughout shifts.
- Clean and sanitize residential properties through daily chores as well as detailed cleaning when needed.
- Clean, fold, iron and care for client laundry and linens.
- Ensure clients are dressed neatly and appropriately according to the weather and activity.
- Provide clients with emotional support and instruction in areas of care, living independently, and adapting to disability.
- Update and review current client records on-site at each property.
- Provide all approved services for the client as scheduled.
- Transport clients to other services, medical appointments, outings and other events as needed.
- Complete goals in accordance with client ISP and track properly.
- Connect client with community resources and natural support system through activities, outings and other events planned by the Case Manager and scheduled by the Property Manager.
- Prepare client meals according to posted weekly menus.
- Protect and care for company and client property to insure long-term use.
- Follow Health Insurance Portability and Accountability Act (HIPAA) privacy rule to ensure client protected health information is secure.
- Ensure that all required certifications remain current including CPR, First Aid, Professional Certification and Licensing, TB clearance and Crisis Prevention Intervention.
- Complete continuing education as required for growth and development.
The ideal candidate will be a self-motivated, compassionate worker that is very attentive to detail and dedicated to making a difference in the lives of our clients.
They will have experience in disability services and will be most dedicated to assisting clients meet care plan goals while helping them maintain the greatest lifestyle possible.
This candidate will also have a very high level of skill in nursing. They must complete a minimum of 16 hours continuing education training annually.
