Sentara Health is hiring for a Nurse Case Manager / Private Duty Nurse / RN- Tidewater, VA!

Standard working hours : 8 am to 5 pm EST, M-F
Location : Remote but will conduct
home visit in members' homes. Need to reside in Tidewater, VA (
Norfolk, Virginia Beach, Suffolk, Portsmouth and Chesapeake, VA).
Nurse Case Managers serving Members in High Intensity Care Management with the following conditions :
Members receiving private duty nursing services
Life-sustaining Ventilator dependent members
Vulnerable infant members, including those diagnosed with neo natal abstinence syndrome, classified substance-exposed, or admitted at NICU Level 3
Job qualifications :
Min 3 years Nursing experience REQUIRED
Registered Nurse / RN License in
Virginia / Multi-state / Compact
REQUIRED
For Integrated Care Management departments, specialty certification REQUIRED within one year of eligibility (ACM, CCM, CCCTM or RN-BC).
For other service lines, certification based on specialty area required within one year of eligibility.
Preferred qualifications :
3 years Case Management experience preferred.
1 year working directly with members who meet the conditions listed below :
a. Members receiving private duty nursing services
b. Life-sustaining Ventilator dependent members
c. Vulnerable infant members, including those diagnosed with neo natal abstinence syndrome, classified substance-exposed, or admitted at NICU Level 3
N-2YR - RN-Associate's Degree
N-4YR - RN-Bachelor's Level Degree
N-3YR - RN-Diploma (Non-degree)
N-DN - RN-Doctorate Level Degree
N-6YR - RN-Master's Level Degree
