Manages the day-to-day coordination, administration, community engagement, and data functions for Community Health Initiatives, under the direction of the RN Regional Director Population Health.
Primary Duties and Responsibilities
Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards.
Complies with governmental and accreditation regulations.
Acts as primary agent for the coordination and implementation of Population and Community Health initiatives including developing policies, procedures, goals, and objectives.
Plans and manages logistics for community events.
- Supports community health needs assessment and community benefit processes.
- Assesses needs; analyzes and coordinates health teaching and education programs for target populations.
- Mentors nursing and allied health students.
- Assists in the synthesis and presentation of data into clear, relevant, and visually appealing materials that can be used to make strategic decisions.
- Participates in grant application processes for sustainability of community health initiatives.
- Supports advocacy and other activities with partners to meet strategic goals.
- Coordinates outreach, research, and other activities with initiative partners to minimize redundancies and to align strategies and actions, including conducting individual outreach and education as needed.
- Creates and manages summary reports of community health programs. Ensures that records are kept in compliance with regulatory agencies requirements.
- Participates in meetings, and on committees, local coalitions, steering groups, and advocacy groups. Represents the department and hospital in community outreach efforts.
- Participates in multi-disciplinary quality and service improvement teams.
- Serves as a resource to colleagues and consumers of healthcare.
- Supervises and provides vaccinations and health screenings, including, but not limited to, vital signs, body mass index, visual / hearing acuity, and carbon monoxide levels.
- Provides program management assistance at regional facilities as assigned. Working at other MedStar sites, departments or programs may be required.
- Performs other duties as assigned.
Minimum Qualifications
Education
- BSN required from accredited program.
- Master's degree in a health-related field from a nationally accredited school / program preferred.
Experience
- 3-4 years Work experience including project management, population health coordination and staff development or continuing education required.
- Experience with data collection and reporting. Community outreach experience required.
Licenses and Certifications
- RN - Registered Nurse - State Licensure and / or Compact State Licensure Valid RN license in the State of Maryland or multistate compact state required.
- CHES - Certified Health Education Specialist preferred.
- MCHES - Master Certified Health Education Specialist preferred.

This position has a hiring range of $41.98 - $65.17.
