Responsibilities :
- Provide direct patient care in accordance with individual care plans
- Assist with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting
- Monitor and record vital signs, such as blood pressure, temperature, and pulse
- Provide Medication Reminders under the supervision of a registered nurse
- Assist with mobility and transfers, ensuring patient safety at all times
- Provide companionship and emotional support to client's
- Maintain accurate and up-to-date documentation of patient care activities
- Communicate effectively with client's their families, and the healthcare team
