Care Coordinator Social Worker II

Location : Hybrid, Santa Barbara, CA office as needed (prefer local to Ventura, SB, or SLO counties)

Description :

The Care Coordination Social Worker II (CCSW II) reports to the Care Coordination Supervisor and works with members to provide psychosocial support and care coordination. This role addresses members’ complex medical and social needs, connects them with community resources, and supports their engagement in Health Services Clinical Programs, including Case Management, Care Coordination, Pediatric-Whole Child Model, Disease Management, and Behavioral Health.

What You Will Do :

  • Conduct assessments and develop member-centered care plans.
  • Follow up with members and providers to monitor progress and adjust care plans.
  • Coordinate services with healthcare providers and community resources.
  • Assist members with transitions of care, housing, or alternative living arrangements.
  • Support members in Disease Management and other clinical programs.
  • Conduct home visits as needed and consult with clinical team members.
  • Maintain an active caseload and comply with HIPAA and mandated reporting.

You Will Be Successful If :

  • You have strong expertise in care management and psychosocial support.
  • You communicate effectively with members, families, and providers.
  • You are self-motivated and able to work collaboratively within a team.
  • You are knowledgeable about social resources and managed care issues.
  • You can manage members across multiple clinical programs efficiently.
  • You are proficient with relevant software and documentation tools.

    • What You Will Bring :

  • Master’s degree in Social Work, Clinical Psychology, or Psychology (required).
  • Minimum 5 years of clinical experience; LCSW may substitute for 2 years.
  • At least 1 year of case management and care coordination experience.
  • Experience with adults, children, or young adults with special needs; experience with two or more groups preferred.
  • Knowledge of managed care, community resources, and crisis intervention preferred.
  • Bilingual in Spanish preferred; Case Management certification a plus.
