RN Case Manager - Memphis (Mon-Fri, 8 AM-4 : 30 PM)

Are you an experienced RN looking for a meaningful role with a balanced schedule? We're seeking an RN Case Manager to join our Memphis team, providing personalized, one-on-one care to patients in their homes.

Position Highlights :

Schedule : Monday-Friday, 8 AM-4 : 30 PM - Enjoy your weekends off!

• Location : Memphis, TN - In-home patient care, no facilities.
• Role : Manage patient care plans, collaborate with interdisciplinary teams, and ensure high-quality care in the comfort of patients' homes.

Qualifications :

• Active RN license in Tennessee
• Case management or home health experience preferred
• Strong organizational and communication skills

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients and families
• Initiate patient plan of care and reevaluate as necessary
• Utilize health assessment data to determine nursing diagnosis
• Administer medication and treatment as prescribed by the physician in the plan of care
• Counsel the patient and family in meeting nursing and related needs
• Complete and maintain accurate clinical notes
• Communicate with the physician and participate in interdisciplinary group meetings to meet the medical needs of the patient
• Ensure compliance with all state and federal legal and regulatory requirements
• Additional responsibilities as assigned

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• 1 year nursing experience required, 3+ years preferred
• Experience in home health or a similar setting preferred
• Commitment to clinical and documentation excellence
