We are hiring Homecare Registered Nurses and Licensed Practical Nurses - full time and per diem for our next orientation starting on October 23, 2024 .
We care for mental health patients of all ages and backgrounds who require daily visits in their home in the surrounding areas :
Position 1 : RN or LPN Full Time AM or PM Shift in Cambridge, Charlestown and the surrounding areas, up to $80K annually plus incentive pay.
Position 2 : RN or LPN Per Diem to work Weekends AM or PM shifts in Cambridge, Charlestown and the surrounding areas, Up to $35 / visit.

To thrive as a Full-Time or Per Diem Registered Nurse or Licensed Practical Nurse at Comfort Home Care, you will need a toolkit of essential skills and a compassionate heart! Performing initial and ongoing assessments is crucial to identifying your client's medical needs and their ability to communicate and learn effectively.
You'll initiate and revise care plans, conducting OASIS assessments as required to ensure personalized and optimal care.
Strong coordination skills are vital, as you'll keep physicians and other personnel informed of any changes in your client's condition and needs.
Collaboration is key in this role, enabling you to work closely with patients, families, and other healthcare providers for effective care planning and discharge processes.
Lastly, a keen attention to detail is essential for preparing clinical and progress notes, ensuring timely submission of all required paperwork according to agency policies.

Knowledge and skills required for the position are :
- RN or LPN provides initial and ongoing assessments to identify the client's medical needs level of functioning and ability to communicate and learn.
- RN or LPN initiates the plan of care necessary revisions and performs OASIS assessments at appropriate time points when applicable.
- RN or LPN initiates and performs appropriate nursing interventions.
- RN or LPN coordinates services informs the physician and other personnel of changes in the client's condition and needs.
- RN or LPN collaborates with patient / family and other health care providers and / or community resources with planning of care and discharge.
- RN or LPN prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
