PATIENT POPULATION :

The patient population served can be all patients, including geriatric, adult, adolescent, pediatric, and newborn. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.

POSITION SUMMARY :

Under the general direction of the Unit Director, Clinical Manager and Charge Nurse, the Registered Nurse (R.N.) independently provides holistic, individualized care using the Nursing Process for patients routinely seen on the unit.

Demonstrates an advanced level of clinical competency, Nursing judgment, teaching skills, and a commitment to improving the quality of patient care.

Independently performs general and unit-specific clinical skills. Contributes to the Sierra View Medical Center Department of Patient Care Services and the Nursing profession in a positive manner.

Is responsible for the assessment, treatment or care appropriate to the ages of the patients served. Demonstrates the ability to obtain and interpret information in terms of the patient's needs.

Possesses knowledge of growth and development and has an understanding of the range of treatment needed for all patient groups.

Must be able to work normal / scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call.

Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act.

LICENSURE / CERTIFICATIONS :

To perform this job successfully, an individual must have current licensure as a Registered Nurse in the state of California.

Must have and maintain current BLS card or obtain prior to hands-on patient care. Experienced NICU nurses must have and maintain NRP upon hire.

If new hire / transfer has no NICU experience, NRP must be obtained within the orientation period; prior to assuming own patient care assignment.

STABLE certification will be required within 6 months of hire date.

Responsibilities and Essential Functions :

  • Is responsible for the assessment, treatment and / or care appropriate to the ages of the patients served. Demonstrates the ability to obtain and interpret information in terms of the patient's needs.

Possesses knowledge of growth and development and has an understanding of the range of treatment needed for all patient groups.

  • Utilizes holistic assessment skills, recognizing subtle, covert or abnormal signs and symptoms that include at least one of the following : involvement of patient / significant other, discharge needs, psychosocial needs, biophysical needs, environmental needs, teaching needs.
  • Identifies nursing diagnosis and interdependent nursing problems and selects applicable nursing care standards to guide patient care.

Recognizes issues / concerns that could impact development of mutual goals.

4 Ensures that the Care Plan is specific and unique to the individual patient utilizing Interdependent Nursing Standards and the discharge needs of complex / multi-system patients.

5 According to established clinical policy, procedure and standards, the R.N. safely performs nursing care including safe administration of medication, troubleshoots equipment and demonstrates skill in clinical problem solving.

6 Ensures that the nursing care provided is consistent with the established Nursing Plan of Care, the Medical Plan of Care and the ages of the patients served.

  • The effectiveness of the Plan of Care is evaluated and communicated through exchange report; recommendations are made for future care and are consistent with current documentation.
  • Communicates evaluation of, or changes in, patient status providing supportive data to the physician or health care team as appropriate.
  • Demonstrates adherence to the Joint Commission National Patient Safety Goals for Hospitals as defined by Scope of Practice.
  • Documentation reflects compliance with established Hospital policies and procedures.
  • Demonstrates knowledge of role by adherence to NPSGs as it relates to area of practice.

12 Demonstrates an understanding of the use, actions and interactions of drugs used in combination for patients with multi-system problems.

  • Completes required unit specific performance criteria checklist.
  • Demonstrates proper Infection Prevention skills to include Standardized Protocols or procedures related to central lines and indwelling Foley catheters.

15 Reports quality or Risk Management issues through appropriate lines of authority, i.e. administrative or clinical.

16 Maintains current knowledge of health care trends that affect the clinical practice of Nursing on the unit.

17 Demonstrates standards of documentation as evidenced by individual chart review and no letters of education in file.

  • Upholds all aspects of the Nursing Practice Act as defined by the California Board of Registered Nursing.
  • Provides evidence of adherence to all nine (9) ANA Code of Ethics for the Nursing profession.

PEDIATRIC DEPARTMENT POSITION SPECIFICS :

1 Demonstrates knowledge of breast-feeding and encourages the mother to breast feed.

2 Is informed of current developments in the nursing field and Maternal Child Health.

3 Is knowledgeable of medications used in the Maternal Child Health Department, including Neonatal dosages. Is knowledgeable of Pediatric / Neonatal dosages when administering medications to the newborn.

4 Performs physical assessment of the newborn by following established policies and procedures.

5 Utilizes appropriate anatomical sites for intramuscular injections.

6 Is competent in assembling and using equipment such as : Oxygen supplies, Oximeter, Radiant Warmer, Neonatal monitor, Suction setups, Catheters, Neonatal emergency supplies, and Bilirubin light.

7 Demonstrates competency in obtaining a Newborn Metabolic Screen.

NICU DEPARTMENT POSITION SPECIFICS :

1 Demonstrates knowledge and skills in performing core competencies for NICU which includes managing Bubble CPAP, Nasal Canula, assisting with Intubation, setting up suction apparatus, oxygen supplies and assisting with UAC / UVC placement.

2 Demonstrates competency on peripheral intravenous insertion and management of intravenous fluids.

3 Is knowledgeable of neonatal medication, dosages and administration.

4 Performs physical assessment of the newborn including use of Ballard and Dubowitz tool.

5 Utilizes appropriate anatomical sites for intramuscular injections.

6 Is competent in assembling and using equipment such as : Radiant Warmer, Isolate, Neonatal monitor, Catheters, Neonatal crash cart, Bilirubin Light and Telemedicine.

7 Demonstrates competency in obtaining a Newborn Metabolic Screen
