Department Registered Nurse (RN) Case Manager in Tyler, TX.

Responsibilities
- Coordinate and advocate for our patients care & create an appropriate discharge plan.
- Review patients charts, and meet with families and healthcare providers to establish the most suitable course of action.
- Should remain in contact with patients and their families during the hospitalization stay.
Qualifications
Job Requirements :
- Current RN license in the state of Texas
- Registered Nurse with an Associates, Bachelor’s or Master’s Degree
- Minimum of three (3) years of clinical experience
Preferred Job Requirements :
- Case Management certification
- Experience in utilization review, discharge planning, quality programs
