About The Position
Job Title : Registered Nurse Job Specialty : Home Health Job Duration : 13 weeks Shift : Day Shift, 5x8-Hour (08 : 00 - 17 : 00) Guaranteed Hours : Not guaranteed Experience : Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience, depending on the specialty License : Must have a valid nursing license in hand Certifications : CPR, BLS Must-Have : - Proficiency in OASIS and HomeCare HomeBase (HCHB) within the last year Job Description : Provides, coordinates, and directs the provision of home nursing care according to physicians' orders.
- Follows agency policies and procedures through the competent application of the nursing process.- Responsible to the Clinical Manager.
- Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.- Must be willing to travel up to 75 miles from the home clinic, with mileage reimbursed at the IRS standard rate.
- Manages an average caseload of 5-8 patients per day.- Covers a territory within a 60-mile radius of Salem, Oregon, including Marion, Polk, Linn, Yamhill, and Clackamas Counties.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.- Required to participate in on-call rotations and potentially some weekend shifts.
- Reports for assignment at a designated address in Salem, Oregon.- Expected to travel approximately 200 miles per week.- Achieves a weekly productivity expectation of 30 points.
- Attendance at periodic office meetings is required.- Must have reliable transportation, a current driver's license, and agency-required liability insurance.
