Duties :
- Develop, implement, evaluate and direct the social service programs of the facility
- Participate in community planning related to the interests of the facility and the services and needs of the resident and family
- Maintain community relationships to help address resident concerns as appropriate
- Perform routine tasks with patient admissions, care plans and discharge planning
- Coordinate response to reports of missing, lost or stolen belongings, along with other grievances
Qualifications :
- Must have, as a minimum, a bachelor’s degree in social work or a bachelor’s degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology
- License in the state of Texas in Clinical Social Work
- Social work experience in a skilled nursing / long term care setting working directly with individuals
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies / personnel and the general public
- Must possess leadership ability and willingness to work harmoniously with and supervise other personnel.
