Overview
The Registered Nurse, On Call provides telephone triage, direct nursing care and prescribed treatments to meet the acute needs of home care residents through the on call system.
Uses critical problem solving and good communication skills to effectively address resident care needs.
Responsibilities
Job Duties Specific to This Position :
Resident / Client Care
Conducts nursing assessments and evaluations for ongoing or episodic illness of residents according to the plan developed by the interdisciplinary team.
Counsels resident families regarding interventions as a solution to physical, emotional, and environmental health problems.
Makes referrals to other services when resident conditions or situations require the service of other professional disciplines or products / services of outside companies or agencies.
Participates in the development and revision of the resident's plan of care as a member of the interdisciplinary team when needed.
Provides instruction and demonstrates nursing techniques to residents and care givers.
Provides on-call coverage on a rotating basis with other nursing staff to ensure consistency in the coordination and scheduling of patient care in resident homes.
Responds promptly to emergency calls and takes appropriate action when on duty.
Compliance
Conducts appropriate nursing assessment as required by the Licensed Agency procedures to facilitate admissions and care planning process for residents.
Corporate Participation
Establishes and maintains cooperative working relationships with other program staff, outside agencies, and provider hospitals.
Keeps current in his / her area of specialization and in emerging and best practices.
Documentation
Maintains accurate and complete documentation and logs of all services provided, according to program procedures and regulatory requirements.
Quality Assurance
Effectively supports Quality Assurance Performance Improvement initiatives through participation as directed.

Qualifications
1-2 years' minimum experience in long term / acute home care.
Graduate of an accredited school of nursing. BSN or BS in related field preferred.
Current NYS license and registration in good standing as a professional nurse.
Possession of current NYS driver's license and dependable vehicle.
NYS certification in PRI and screening (preferred).
Current knowledge of best practice nursing standards and care.
Excellent assessment and care planning skills.
Outstanding interpersonal and communication skills including the ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.
Ability to work independently and as part of the interdisciplinary team.
Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes.
Effective conflict resolution skills.
Proficient in MS Office programs and able to learn new software rapidly.
An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
