Position : Hospice Response RN Faith Hospice
Schedule : Full-time 12hr nights
Territory : Kent County and the outskirts of surrounding counties
Sign-On Bonus : $5,000
Faith Hospice offers a unique opportunity for an experienced RN to join our dedicated interdisciplinary team. As a Hospice Response RN, you will play a crucial role in providing compassionate care to patients at the end of life.

Responsibilities :
- Respond to crisis calls and patient / family concerns
- Provide end of life visits
- Provide after-hours support and care
- If no visits are needed in the community, assist care team at our inpatient hospice residence called Trillium Woods
- Schedule : 9pm - 9am, 2 weeknights plus every other Friday & Saturday
Location :
Grand Rapids, covering a 35-mile radius around downtown Grand Rapids
Qualifications :
- RN licensure in Michigan
- Minimum 2 years of med / surg or acute care nursing experience
- Hospice or hospital experience preferred
- Passion for hospice nursing
- Valid Michigan driver's license, auto insurance, and reliable means of transportation for home visits
