Job Summary : The Licensed Practical Nurse (LPN) provides general nursing care to patients, families and caregivers in the Medical Group practices.
The LPN assists and advocates for the patient and families health in support of the patient centered approach and delivery model of care.
Responsibilities include collaborating with physicians / providers, registered professional nurses, and multidisciplinary care team members.
This role operates in accordance with the standards outlined for Licensed Practical Nurse (LPN) in the NYS Education Law and State Nurse Practice Act.

- Gather patient measurements and collect data
- Record and report patient findings
- Prepare patient for examinations and treatments
- Assist with examinations and treatments
- Support self-care and the established plan of care
- Utilize standard precautions at all times
- Maintain equipment, supplies, inventory, and a safe work environment
- Engage in quality improvement initiatives and maintain technical knowledge
- Maintain a cooperative relationship among the healthcare team
- Respond to patient requests and communicate effectively with the healthcare provider
Education / Experience Requirements : Graduate of an accredited school of licensed practical nursing program. Preferred : Bachelors or Masters of Science in Nursing from an accredited nursing program.
Licensure / Certification Requirements : Current New York State licensure as a Licensed Practical Nurse; Basic Life Support Certification - Must receive BLS certification within 6 weeks of hire date.
