Overview :

Provides case management for patients and families to enhance their psychosocial functioning. Performs duties and responsibilities in a manner consistent with mission, values and Mercy Service Standards.

Qualifications : Education :

Master's degree required in Social Work, Counseling or other human service related field

Experience :

Minimum one year Behavioral Health experience preferred

Licensure :

Licensed Master's Social Worker (LMSW), Provisional Licensed Professional Counselor (PLPC) or eligibility for licensure preferred

Other :

Co-worker must be able to communicate orally, in writing and demonstrate proficient computer skills. Co-worker must be able to listen and to understand and explain complex situations.
