• Required Certifications : MSW, State licensed
• Required Skills : Outpatient Clinic, Develop plan of care to meet patient / caregiver needs, Documentation of patient / caregiver teaching / learning, Documents patient response, Medical Transportation, Care coordination

Job Details :

• Pay : $2372.85 / week - $2565.65 / week
• Assignment Length : 13-week assignment
• Shift : 5, 8-Hour 08 : 00 - 16 : 00
• At least one year of experience required
