We are looking for a  Physical Therapist  to join the team at  Horizon Healthcare and Retirement Community in Eckert, CO !   At  Pro Rehab , you'll be part of a supportive, mission-driven team that takes pride in making a difference every day! Position Type :   Full-Time

Wage Range :   $43 - $62 / hour depending on experience

Bonus :   $10,000

Location :   11411 CO-65, Eckert, CO 81418

Physical Therapist Responsibilities :

• Assess & Diagnose :  Evaluate patients' physical conditions, movement abilities, and medical history to develop personalized treatment plans.
• Develop & Implement Treatment Plans :   Create and execute rehabilitation programs to improve the functional deficits of the patient.
• Provide Hands-on Therapy :  Perform manual therapy, therapeutic exercises, and use modalities such as but not limited to ultrasound and electrical stimulation as needed.
• Educate & Support Patients :  Provide patient, caregiver, staff education as deemed clinically necessary such as but not limited to : pain management techniques and fall risk safety. Teach patients and families about exercises, injury prevention, and pain management techniques.
• Monitor Progress & Adjust Plans :  Track patient outcomes, document progress, and modify treatment plans as needed.
• Collaborate with Healthcare Teams :  Work with doctors, nurses, and other therapists to ensure comprehensive patient care.
