Advanced Practice Provider (Nurse Practitioner or Physician Assistant)
We are looking for experienced Nurse Practitioners and Physician Assistants to join our growing team providing in-home healthcare services.
As an Advanced Practice Provider, you will work with a variety of acute care cases and provide comprehensive assessments, diagnosis, treatment, and outcome management for patients of all ages.
Our innovative model allows us to deliver definitive care in the home, including minor procedures, wound care, and medication administration.
What You'll Do :
- Perform comprehensive health assessments and diagnose complex illnesses
- Provide therapeutic interventions and minor procedures
- Use critical thinking skills and follow evidence-based standards of practice
- Educate patients and families on treatment and managing health
- Communicate effectively with patients, families, and other healthcare professionals
What You Need :
- Minimum of two years of clinical experience in relevant areas
- Graduate with an advanced degree from an accredited institution
- Current unrestricted state license as a Nurse Practitioner or Physician Assistant
- Nationally board certified and current BLS required
- Prescriptive authority and DEA registration
- Valid driver's license with clean driving record
- Ability to lift and carry equipment up to 50 pounds
