Adult & Child Health is an integrated healthcare provider in Central Indiana. We are a Certified Community Behavioral Health Center and a Federally Qualified Health Center look alike;
we strive to provide the best in healthcare services. Providing high-quality, affordable care including primary care, behavioral health services, substance abuse treatment, mobile crisis response, community-based services, homeless outreach, school-based treatment, and more.
About this role : Health care can be a powerful tool to help an individual become their happiest, healthiest self. However, a person can only benefit from the health care system if they can access and navigate the often-complicated health care system and have their needs and wishes addressed by their providers.
Many clients who come to A&C have significant difficulty accessing care and advocating for their wishes in treatment, and our Care Coordinators work closely through regular contact with them and their providers, both within A&C and at external agencies, to help them get the most out of their treatment.
The Location and hours : A&C is based in Central Indiana. For this position, we service the following county : Marion
Hours are M-F 8-4pm or 9-5pm with flexible scheduling available.
Responsibilities include but not limited to :
- Serve as the primary point of contact for Care Coordination clients on your caseload, assisting them with improved access to integrated health care and client advocacy within their treatment.
- Improving access involves creativity and flexibility in identifying barriers to accessing care, connecting clients with transportation resources as needed, motivational interviewing techniques to help with engagement issues, rescheduling missed or cancelled appointments, facilitating referrals to external providers, etc.
- Improving advocacy involves assessing client satisfaction with care, advocating for clients’ wishes during treatment, and encouraging clients to advocate for their own wishes to their providers.
- Coordinate the flow of information between clients’ various providers, including both those within A&C and any external providers, through regular formal and informal contact (i.
e., staffing's, treatment plan reviews, impromptu meetings, email correspondence, etc.).
Perform regular chart review to ensure that clients’ treatment plans are being implemented as dictated by the supervising psychologist and in accordance with the client’s preferences as clinically appropriate.
Requirements :
- Bachelor's degree in behavioral health or related field
- Must hold a valid driver's license with acceptable driving record and provide own, insured, transportation
- Knowledgeable of Medicaid / Medicare guidelines, Marketplace plans, and commercial insurance benefits and services, preferred
