Work Location : Sylvester Cancer Center
1. The RN is responsible to for the direct care of his / her assigned patients, and to perform a variety of duties in an office setting to support optimum care to our patients.
2. Manage prescription refill and lab line, answering patient inquiries; enter refills for sign off and Uchart messages to appropriate clinical care staff member or physician.
3. Gathers medical documentation for medical necessity of those medications insurances refuse to cover. This requires writing of medical necessity letters.
RN will work with nursing staff, PA’s and physicians as necessary to get missing documentation, and submit prior authorization documentation to the insurance company for medications.
4. Track labs and pathology for patients sending reports to Physician for review.
5. Triaging patient call. Assures patients are called back and their problems managed.
6. Coordinates the scheduling of patients for tests, eg MRI, CT, EGD, EUS, and other GI procedures, and completes required forms for lab work.
7. Provides patients with basic instruction and information regarding tests and GI procedures including how to prep for procedures
8. Maintains accurate and complete patient records / documentation.
- Performs other duties as assigned.
- As necessary, employee may be asked to assist in clinics and obtains vital signs and patient information for inclusion in patient’s medical record.
9. As necessary, employee may assist with physical examinations by preparing supplies and handing instruments / materials to the doctor as directed.
10. Documents in Uchart all patient calls and communication between RN and patient.
11. Coordinates the plan of care with patients / family.
12. Promotes patient safety.
13. Communicates changes in plan of care to the patient / family and the health care team members in a timely manner.
14. Assure Press Gainey scores are held at the expected percents per guideline
15. Reviews all records, lab data, x-rays, and diagnostic procedures relevant to the patient. Communicates information with physician assigned to the patient.
16. Participates in hospital risk management program by reporting all untoward events according to hospital policy in a timely manner.
17. Identify and respond to the educational needs of the patient and family.
18. Assures patient compliance with laboratory and physician visits.
19. Maintains an updated excel spreadsheet with patient clinical data
20. Notifies Clinical care coordinator of patient no show or cancellations.
21. The RN is responsible to for the direct care of his / her assigned patients, to perform infusions in 4-bed Infusion center within Crohns & Colitis clinic.
22. Completes and passes ONS certification. Maintains active certification.
23. Completes and successfully passes VAD training course.
24. Completes and successfully passes Infusion training orientation.
25. Verifies chemotherapy / biotherapy consent for treatment is signed and validated
26. Coordinates time of administration with pharmacy and others as needed
27. States knowledge of drug and associates nursing implications
28. Checks that laboratory values are within acceptable parameters and reports results to MD / NP as needed
29. Identifies if agent vesicant or irritant
30. Verifies adequacy of venous access and appropriate IV-site selection
31. Verifies that original order is entered correctly
32. Verifies accurate height and weight and recalculates BSA and drug doses.
33. Verifies Cr. Clearance, formula utilized and recalculates carboplatin AUC dosage
34. Engages a second chemotherapy competent nurse recalculate dosage
35. Verifies that dose is within 10% variance range and initiates intervention if beyond 10%
36. Checks chemotherapy order for drug, doses, schedule, and route.
37. Verifies documentation of dose modifications
38. Ensures copy nonstandard regime, protocol. or of abstract is available
39. Verifies the presence of appropriate prehydration orders and specified parameters are indicated
40. Verifies initial patient education has been provided
41. Verifies patient understanding of treatment plan
42. Applies gloves and gown and uses safe-handling precautions
43. Compares original order with delivered drug
44. Verifies patient identification and shows chemotherapy label to patient
45. Verifies with second chemotherapy competent nurse the accuracy of volume infusion pump settings
46. Verifies adequacy and patency of venous access flushes line with 5-10 cc NS.
47. Demonstrates safe IV free flow administration :
a. Administers through side arm, or at hub closer to patient
b. Administers slowly rate 1-2 ml / minute, unless otherwise indicated
c. Vesicant verify blood return every 2-5ml
d. Demonstrates safe IV continuous infusion administration
e. Verifies use only central line for vesicant continuous infusions
48. Flushes with compatible solute between administration of medications
49. Demonstrates appropriate monitoring / observation for specific acute drug effects
50. Verbalizes appropriate action in the event of extravasation
51. Flushes line with at least 5-10cc NS
52. Appropriately removes device or flushes / maintains VAD
53. Disposes of chemotherapy waste according, and patient response
54. Documents medications, education, and patient response
55. Communicates post treatment considerations to patient and family members
56. Assures all chairs and equipment (IV pump, pillows, desk, chair)are wiped clean, lowers footrest and restocks areas with scrub the hub boxes, gloves, 2x2, alcohol wipes, bandaids, after each patient use and at the end of the day and prior to leaving for the day.
57. Provides patient with refreshments / meal as needed
58. Assures all medications are taken down prior to leaving for the day
59. Teams up with Clinic RN to assure Crash cart log is completed daily
60. Reviews patient uchart notes prior to infusions
61. Assures soiled linen room is always locked, and clutter free. Bins are not overfilled and only 1 bin is to be used at a time (not stacked together).
62. Assures all patients are scheduled for their next infusion appointment and receive AVS.
63. Provides hand-off communication before lunch and at end of day to clinic RN
MINIMUM QUALIFICATIONS
Education :
Graduate from an accredited school of nursing, Bachelor’s degree (BSN) preferred. RNs hired with an Associates (ASN) degree have 2 years to complete the BSN degree.
Certification and Licensing :
Valid Florida Registered Nurse License, ACLS, BLS certification from the American Heart Association required.
Experience :
Minimum 2 years of nursing experience
