JOB PURPOSE :
The incumbent will provide case management services to clients in our Health & Dwelling Medical Respite Program. The Health & Dwelling Program is located in our emergency shelter and serves up to 25 clients who have recuperative care needs.
The Medical Respite Case Manager provides housing focused case management services with the goal of moving clients to stable housing with improved health and wellness.
The Medical Respite Case Manager assesses the needs of each participant in the program, creates housing plans and contributes to the development of the client Care Plan that guides the multi-disciplinary service delivery.
ESSENTIAL JOB DUTIES
- Provides comprehensive, structured, and trauma-informed case management support, coordination of care, documentation of progress, service tracking and outcomes evaluation, for participants in a timely, organized, and accurate manner.
- Meet with clients in the medical respite program regularly to facilitate housing and care plans working towards program discharge.
- Works with clients to conduct an individual needs assessment and care plan.
- Assists the medical respite team in care coordination activities including appointments, transportation, and obtaining medications and medical supplies.
- Works cooperatively / proactively with healthcare workers, case managers, clinicians, housing navigators, and shelter staff to help clients reach their goals.
- Screens clients for mainstream benefits and resources and links clients to Medicaid, SNAP benefits, Social Security and other benefits as needed.
- Obtain resources needed to transition into housing (e.g. documentation / identification, income).
- Provide referrals to supportive services in the community and coordinates activities with other agencies such as Charles Drew Health Center.
- Develop discharge and transition plans with the medical respite team as part of the care plan.
- Completes all documentation and data entry as required, in a timely manner (Progress notes, Homeless Management Information System, Care Plans etc.).
- Provides excellent internal and external customer service, behaving in a manner that cultivates collaboration, respect, and teamwork.
- Implements agency policies and procedures to effectively serve clients in the program.
- Participate in applicable meetings, trainings, conferences, etc. as required, both internally and externally.
- Follow MACCH CoC workflows and training guidelines and ensure compliance with data standards and record-keeping as required internally and by external stakeholders.
- Communicate effectively in oral and written forms with clients, their families, coworkers, supervisors, other service agencies and the community.
- Provide culturally sensitive services and be respectful in all client and staff interactions.
- Adhere to all client confidentiality requirements & standards.
- Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
- Perform other duties as assigned.
JOB REQUIREMENTS :
- Knowledge of case management practice and proficient in the competencies of case management delivery
- Knowledge of issues surrounding chronic health care conditions, mental illness, domestic violence, chemical addiction, Housing First, low barrier service delivery trauma informed care, motivational interviewing and conflict resolution.
- Flexibility to deal effectively with a variety of people, situations, problems and changes.
- Possess knowledge of local community agencies, programs and available client resources.
- Ability to work with diverse populations and all levels of people within the workplace.
- Ability to work independently in a fast-paced environment and be organized with extreme attention to detail.
- Continued development of resiliency skills and practice.
- Maintain and update system of information related to available services and programs.
EDUCATION & EXPERIENCE REQUIREMENTS
- Bachelor’s Degree in health care, social work, sociology, psychology or related field is required.
- Two years medical social work experience working is preferred.
- Excellent communication, verbal and written, and interpersonal skills.
- Two years employment experience in medical case management preferred.
- Basic working knowledge of Windows (MSWord, Excel and publisher), e-mail and Internet.
