What you'll do :

• Conduct outreach to patients from diverse populations to engage them in population health initiatives
• Build rapport and establish trusting relationships with patients to promote their active participation in care
• Identify barriers to healthcare access and address social determinants of health that impact patient well-being
• Educate patients on available resources, preventive measures, and self-care strategies
• Perform comprehensive medical and behavioral assessments to evaluate patients' physical, psychological, and social needs
• Identify and address mental health issues, substance abuse, domestic violence, and other psychosocial factors
• Collaborate with healthcare providers to develop individualized care plans based on assessment findings
• Advocate for patients' needs and preferences in care planning and decision-making processes
• Collaborate closely with a multi-disciplinary team, including physicians, nurses, care coordinators, and community partners
• Participate in care conferences and team meetings to discuss patient cases and develop integrated care plans
• Share expertise on social determinants of health and provide guidance to the team on addressing patients' social and emotional needs
• Coordinate referrals to community resources, mental health services, and support groups
• Maintain accurate and up-to-date documentation of patient interactions, assessments, and care plans
• Adhere to privacy regulations and ensure the confidentiality of patient information
• Provide timely and thorough reports to the multi-disciplinary team regarding patient progress and interventions
• Collaborate with healthcare providers to ensure continuity of care and coordination of service

Requirements :

• Master's degree in Social Work (MSW) from an accredited institution
• Current and valid licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW)
• Minimum of 2 years of experience as a social worker, preferably in a healthcare or population health setting
• Knowledge of medical and behavioral assessment techniques, mental health interventions, and crisis management
• Strong understanding of social determinants of health and their impact on patient well-being
• Excellent communication and interpersonal skills to engage patients and collaborate with a diverse team
• Demonstrated ability to work collaboratively in a multi-disciplinary team environment
• Strong interpersonal skills to build effective relationships with colleagues, patients, and community partners
• Excellent problem-solving and conflict resolution skills
