About The Position
Job Title : Rehabilitation Therapist Job Specialty : Physical Therapist Job Duration : 13 weeks Shift : Monday to Friday, Days Guaranteed Hours : 32 hours per week Experience : SNF experience preferred;
New graduates welcome License : State license verification required Certifications : Not specified Must-Have : Completion of prescreen, resume, and license verificationJob Description : - Conduct assessments and develop personalized treatment plans for patients.
- Provide physical therapy services to improve patients' mobility and physical function.- Document patient progress and adjust treatments as necessary.
- Collaborate with other healthcare professionals to ensure comprehensive care.- Ensure compliance with safety standards and regulations.
- Administer Covid-19 tests 72 hours prior to the start date with results available 48 hours before the start date.- Complete the following credentialing checks : - State License Verification - Resume - Three references - County, Federal, and Multi-State Criminal Search (7 years or more) - Nationwide Sex Offender Search (7 years or more) - HHS / OIG Search - GSA Search - Social Security Number Trace - 1 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray - SAM Search - DOJC (rerun every 4 years with BID form) - No holiday coverage required.
