Job Description :

The Behavioral Health Care Manager will be responsible for reviewing patient charts and communicating with the Clinical department and providers around any irregularities as part of chronic care management.

Responsibilities :

• Provides assessment and care management services, including :
• The administration of validated rating scales
• Initiates behavioral health care planning concerning behavioral or psychiatric health problems
• Revises / modifies care plans for patients not progressing or whose status changes
• Brief psychosocial interventions if needed
• Ongoing collaboration with the billing practitioner
• Maintenance of the registry / tracking sheet C
• Consultation with the psychiatric consultant
• Has a continuous relationship with the patient
• Collaborative, integrated relationship with the rest of the care team
• Conducts an interdisciplinary care plan meeting to go over the patient’s beneficiary to the program

Qualifications :

• Bachelors degree in social work
• Experience in long-term care preferred
• Experience in behavioral health preferred

Salary : $60,000 - $75,000
