What We’re Looking For :
- Education : Graduate of an accredited physical therapy program.
- Licensure : Current, unrestricted physical therapy license.
- Experience : Minimum of one year of clinical experience.
- Credentials : BLS or CPR-First Aid Certification as required, with current TB 2 Step, Quantiferon, or CXR.
- Skills : Expertise in developing and implementing patient care plans, strong documentation and communication skills, and a commitment to ethical and professional standards.
Home Health experience and proficiency with Oasis Charting are a plus.
Your Day-to-Day :
- Develop and Adjust Care Plans : Create and modify individualized therapy plans tailored to patient needs.
- Effective Documentation : Maintain accurate records of therapy services and communicate effectively with the healthcare team.
- Provide Therapy : Alleviate pain and enhance functional performance through targeted physical therapy.
- Prepare Discharge Summaries : Plan and provide clear discharge instructions and summaries.
- Cultural Sensitivity : Deliver care that respects patient diversity and promotes dignity.
- Continuous Learning : Stay updated with continuing education and adhere to state-specific requirements.
