• Qualifications*
• Physical Therapy License required by the State of Michigan
• Valid Driver's License
• Current Auto Insurance
• Reliable Transportation
• 2 year+ experience working as a Physical Therapist
• Responsibilities*
• Provides skilled physical therapy in accordance with the physician's plan of care
• Assists the physician in assessing the client's functional level by applying appropriate tests and assists in the development and revision of the plan of care, including OASIS assessments and goal setting as appropriate
• Treats clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy interventions stated in the physical therapy plan of care
• Teaching and promoting disease prevention and injury management for enhanced patient and caregiver independence
• Assists with the coordination of the total plan of care and maintains continuity of client care by liaising with other health professionals assigned to the same clients.
