CASE MANAGER (TITLE III E PROGRAM, SENIOR SERVICES)

We are now hiring for a full-time Title III E Program, Senior Services Case Manager to join our Team at Southeast !
SALARY : Compensation is commensurate with years of related experience, position requirements, and candidate qualifications.
The average salary for the position is $45,571.
ESSENTIAL JOB FUNCTIONS :
- Conducts thorough assessment of clients. Evaluates problem and service needs
- Develops a written client service plan specifying goals to be achieved and services to be pursued.
- Supplements service plans to meet client needs.
- Provides, as appropriate, supportive help, advocacy and case management to individuals and families in coping with environment and interpersonal relationship issues.
- Works collaboratively with community resources. Develops a knowledge of resources and helps clients to utilize them.
- Works within contract, grant, and program requirements including meeting timelines and reporting requirements.
- Reassess and monitors client situation and adequacy of service plan.
- Collaborates and participates with other program staff in the planning and implementation of service plans.
- Subsequent to consultation with supervisor, handles emergency and crisis situations within target population.
- With supervisory approval, authorizes limited amounts of financial assistance to own clients.
- Assists clients considered most at risk by providing one-on-one therapeutic counseling sessions for the targeted population, family sessions and / or tailored training for individual clients or their family members.
- Conducts one-on-one and group outreach for the purpose of identifying potential clients and encouraging use of the agency's services and benefits.
- Maintains competencies in working with diverse populations to successfully engage clients in assessments, care planning, and referral and linkage.
- Completes documentation / data entry on services rendered, attempted contacts, staffings, supervision, etc. in EHR, health population platforms, and / or funder required data bases in a timely manner.
- Completes and / or runs reports specific to populations served to ensure tasks are completed in a timely manner.
- Required to work beyond traditional work hours, including late nights and weekends, to meet the needs of the program.
QUALIFICATIONS :
- Bachelor's Degree in Social Work or related field from an accredited college or university, required .
- One year experience in social service provision to older persons required .
- Computer competency sufficient to navigate between record keeping systems, required .
- Well-developed clinical skills (formal clinical degree or significant clinical job experience), including counseling and support group facilitation, required .
- Working knowledge of technology and assistive devices related to caregiving.
- Strong interpersonal and time management skills.
- Some direct service experience preferred.
- Requires mobility to work with a community based caseload.
- Ability to maintain assigned community and home-based services.
- Ability to work effectively with diverse individuals and groups.
- Working knowledge of the Illinois Department on Aging (IDoA) Adult Protective Services program and resources and programs available for older adults and people with disabilities are preferred .
PHYSICAL DEMANDS
- While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch.
- The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing and viewing a computer terminal.
- The employee may occasionally lift and / or move up to 50 pounds.
