Essential Job Functions
- Participate in the medical evaluation of patients admitted at the Skilled Nursing Facility.
- Evaluate and document medical necessity for specific level of care. If patient doesn’t qualify for specific level care, facilitate transfer to appropriate level of care.
- Offer advice and information to staff and referring physicians on medical interventions consistent with the medical plan of care.
- Consult with attending physicians, and referral sources as requested, so comprehensive medical care for patients is effectively and expeditiously accomplished.
- Provide information to patients and families regarding their medical condition and plan of care.
- Participate in the review and development of practice protocols in the facility.
- Responsible for informing the primary physicians and referral sources the outcome of the Nurse Practitioner consultations and evaluations.
- Responsible for documenting interactions between nursing staff, referral sources, patients and families.
- Maintain regular contact with referral sources as assigned.
- Complete, maintain and submit accurate and relevant clinical notes regarding patient’s condition and care.
- Electronic medical record documentation is an expectation of all clinical personnel for quality care, accuracy, and communication standards.
The clinician is expected to appropriately document in the electronic medical record, have timely synchronization of patient data & effectively communicate with the Skilled Nursing Facility.
- Present education and information to members of the SNF care team and / or medical community as needed.
- Participate in the resolution of interpersonal conflict and issues of clinical and ethical concern as needed.
- Participate in research activities or special projects as assigned and needed.
- Participate in call schedule as per other members of call team (compensated separately)
- Other duties as assigned.
Qualifications
Minimum qualifications will be considered required unless specifically stated otherwise)
Experience :
- Minimum of two (2) year's of professional experience in health care.
- Experience in a skilled nursing facility preferred.
- Experience in acute care in an institutional setting preferred.
Education and Training :
Acquired through formal education, outside study, training on jobs of lesser degree, or by any combination of these. May be expressed in terms of formal educational equivalents) :
Nurse Practitioner or Physician Assistant licensure is required.
License, Certification & Registration :
- Possesses and maintains current CPR Certification.
- Current Massachusetts licensure : Nurse Practitioner or Physician Assistant
- Valid driver’s license
Other Requirements :
- Demonstrates good verbal and written communication, and organization skills.
- Strong observation skills and problem solving skills.
- Experience working with an Interdisciplinary Team preferred.
Working Conditions

Working Conditions :
May be exposed to infections and contagious diseases. Contact with patients under a wide variety of circumstances. May be exposed to / occasionally exposed to patient elements.
Subject to varying and unpredictable situations. Handles emergency or crisis situations. Travel required. OSHA exposure category :
- OSHA exposure category : Category I
- Category I Position includes tasks that involve exposure to blood, body fluids, tissues.
- Category II Position includes tasks that involve no exposure to blood, body fluids, tissues, but employment may require unplanned Category I tasks.
